2012 Membership Application
The Membership Application includes the 2012 CFC Application, HCROA Affiliation and Fee Policy Form, and State Campaign Registration Form.

Guidance on Attachment A
We strongly encourage you to review the above for help with your Attachment A. Provided are examples of “do’s” and “don’ts”. If additional help is needed or you are unsure how you should present your description of services, feel free to call us. 

Pro Forma 990 Template
If you filed a 990-EZ, you will need to include a “Pro Forma” 990 with your application. Instructions are included in the above document.

HOW TO SEND APPLICATION DOCUMENTS:

• The 3-page completed CFC Application Form and Certifications must be submitted as hard copy with original signature. Faxed and emailed copies of this document will not be accepted.
  
• Attachment A is preferred as an electronic document – either Word, Excel or a combination of the two. This will make it easier for our staff to help you if revisions are recommended.
  
• All other attachments to the application may be sent in whatever format is most convenient to you. We strongly encourage electronic submissions.
